Health Insurance Billing Provided:
We are happy to bill your insurance provider for services rendered. Please verify that Carolyn Durham is on your list. If your plan covers Massage Therapy and we are not on your list, let us know and we would be happy to become an approved provider.
Workers Compensation Claims:
We are happy to work with your Workers Compensation Company to process the billing and provide progress notes. A referral and diagnosis is required along with approval from your company.
